Tuition Rates for 2008

            St. Francis of Assisi School Admission Checklist

I.  Required Forms

v     Application Form (includes emergency information and child’s social

                                                      security number)

v     C&E Lockbox Services ( Tuition Management Program Form)

v     Copy of Current Immunizations

v     Completion of Health History Form

v     Copy of Child’s Birth Certificate

v     Copy of Child’s Baptismal Certificate, if applicable

II.  Fees

v     Non-refundable Registration Fee of $100, if paid by June 10

v     Activity Fee

§         $60. per child

§         Due by September 30

v     Lunch Fee

§         $45. per family helps to pay stipend for lunch monitors

§         Due by September 30

III.  Optional Fees

v     CARES Registration Fee, if interested

IV.  Requirements

v     Children must meet the age requirement by September 1^st for:

§         Pre-K:  4 years old

§         Kindergarten: 5 years old

§         Grade 1: 6 years old

§         Met All Financial Responsibilities from Previous School

§         Rectory Letter – Active Parishioner in a Catholic Parish

Please request from the rectory at least 1 week in advance.
